Yup, the runway was I believe weeks away from the repair schedule.

Ouch, I wonder how many props suffered damage from this over the years? Weight isn't much of a factor here. A loose surface and a high thrust setting will cause this.
Props will pull up loose debris from just ahead and cause all sorts of nicks and other damage to the leading edge. After one too many repairs the prop will have to go into the trash. A freshly painted prop will look fairly beat up after 300 hours of operation.

Agreed. Weight and plane weren't really a factor here. As was said before, this was purely unsatisfactory pavement failing under normal circumstances. I've landed on bad runways, but I would have never landed on that runway becuase there were very clear "bubbles" in the runway that would have me finding new digs for my plane until the pavement got fixed. That second section of pavement that was pushed up on the takeoff run was a serious hazard because as he hit it, you could see the nose gear strut bottom out and that for me in a prop would've been a no-go condition as it would've been a likely prop strike.

Jet blast at high power is rather poisonous to surfaces. While mass flows are often not that much, the velocities immediately after the engine are quite extreme (in range of 300 m/s) and the blast, before it dissolves into turbulent flow, has this property of digging under anything it find not solid enough. I'd seriously avoid jet operations on runways that are not in shape.
It of course applies to ramps as well..

Agreed Crazac, some of it must have hit the tail section at some point, as the jet exhaust is in front of it.
Question is though, why on earth did they use that runway for take off, and especially in a jet!? Surely they must have known that could be an issue, and why didn't anyone contact the pilot as he was backtracking to tell him he was already ripping the surface up!
As for the weight, i just thought that she used up a hell of a lot of that runway, and did look a little tail heavy anyway. Plus the gear was soon retracted, as if maybe a little anxious about high enough airspeed?
