Take a look at Sim Physics X (or P3D). I've been flying with it for a couple years and it adds a nice degree of realism. (I don't use any of the other features in the package preferring PrecipitFX for rain and snow textures and, of course Accu-Sim.

Try using Process Monitor in Task manager and see if anything goes nuts when your frame-rate drops? Or try the more capable and also free from Microsoft Process Explorer?

Tried headphones, but they don't change the volume much, just reduce some of the high freq noise.
