i have brought the Spitfire and the P-51 with accu-sim at the same time, What i don't understand is there are three packages.

Wings of Silver B377 Stratocruiser
Starting at: $36.99

Accu-Sim Captain of the Ship for the B377
$29.99

Accu-Sim Captain of the Ship for the B377 (UPGRADE)
$19.99

What do i get, and in what order.

The upgrade is for people who previously purchased Accu-Sim for the 377. You do not need this. Captain of the Ship contains the newest versions of Accu-Sim. You need to purchase the base package 377 and CotS.
