Good story. I saw a documentary where a 767 had to make a dead stick because of fuel starvation. Another great story. The guys fueling the airplane had a lbs vs Kilo mix up and no one caught it. They still can't get the pucker marks out of the cockpit seats. :D

Good story, nice reminder to be vigilant with aviation. Not exactly sure where they got 9 miles high, since that's close to 50,000 feet, though.

I believe that's a reference to Part II Stefan, which is apparently an article about the Space Shuttle Challenger disaster. The account linked by Jetranger is the "Twelve Mistakes" bit.
